Building Stronger Families. Child, Family, Marriage & Individual Psychotherapy.
My Pledge for Great Care:
I pledge to provide continuous communication with Primary Care Physicians, Family Physicians, Neurologists, Psychiatrists, and all other care providers in order to provide our mutual patients with the best continuation of care.
When a patient registers for services with my office, we ask them to complete and sign a release of information allowing communication to begin with our initial evaluation. As long as the member signs the release a letter is then generated acknowledging the results of our initial evaluation and its diagnostic findings.
The information forwarded to the patients chosen care provider consists of the initial date of service, the diagnosis and the place of service. If care providers are medicating a patient then contact can be by telephone and/or letter. With the authorized care providers input more complicated cases are referred to specialized providers (Psychiatrist or Neurologists).
In a world with fast paced managed care companies I believe it is essential for all providers of patient care to communicate on a regular basis. In keeping my pledge I routinely send out updates especially when there has been a change in treatment.
I enjoy the relationships I have established over the years with all of the different care providers. Each year I attempt to add to the list of Care Providers who utilize my services.
Sincerely,
Ira L. Bilofsky
